It is a well-known fact that most of the doctors follow the traditional way of handwritten medical prescription. Further, the diagnostic and medical record that every hospital, doctor or laboratory provides are different, leading to having many different medical records in a patient's hands. Whenever there is a need for a patient to go to hospital or diagnostic laboratory, a patient is required to retrieve a medical record, which is a very cumbersome thing on part of the patient. Many times, the handwritten prescriptions and medical records are lost and even electronic records are difficult to search , Though the patient’s medical history is the most important aspect for assessing and analyzing his present health condition, compatible treatments, response to medicines and progress shown by patient to specific treatments.

There are many platforms for electronic health record on part of laboratories and doctors but no platform is available for patients to keep record of their medical prescription, diagnostic reports and analyzing them so that the patient can change health behaviour based on such analysis.

My Health Vault (Pending Patent) relates to personal health record and management system for storage, sorting and analyzing the health records and retrieves the records and information according to requirement. The present invention also provides a method for storage, sorting and analyzing the health records and retrieves the records and information according to user requirement.

2 The system has facility for shorting of prescriptions, lab reports and clinical findings either Individually or combination of the following parameters.
a. On the basis of period.
b. On the basis doctor name or hospital name.
c. On the basis Lab name.
d. On the basis doctor’s speciality.

3 The system has also a facility to the patient, if the changes in the health parameters are updated by the patient in the system during the course of treatment, the system will notify the changes in medication and diet, if any, to the patient on real-time basis.
