Care4Today® Connect is designed to help empower you to take a more active role in your health, with helpful medication and appointment reminders and tracking capabilities for key health measures. This application can help you track your progress and provide support and motivation in between healthcare visits in the following ways:
• Delivers medication, medication refill, and appointment reminders that you set up to help you stay on track.
• Records information you enter such as blood pressure, exercise, mood, etc. and tracks those reported measurements to help you see your trends
• Allows you to link external fitness apps like Apple Health to pull in your data around sleep, step count, and more
• Enables you to share graphs and trends on your self-reported data with your healthcare professional allowing for more productive conversations during healthcare visits
• Provides access to medications, appointments, and self-directed adherence data and organizes it all in one place
